Community Integration and Medication Management
“I think Common Ties is a wonderful program. The services I receive have made a difference in my life.”
- Client
Our Community Integration team works with adults living with mental illnesses, and who are often challenged with a co-occurring substance use illness, to develop an individual plan to access resources, improve skills, and manage symptoms of the illness. We also help develop daily living and housing supports, as well as employment, education, psychiatric, and counseling services to meet your needs and achieve your individual goals.
As you know, the path to wellness has its ups and downs. Our team is here for you to support you during the good times and the challenging ones.
Our Intake Team will assist you to determine your eligibility for our programs. Please call or visit our office to find out more information and take the first step toward identifying and achieving your own personal goals.
Employment Specialist Services
Employment Specialist Services are now available through both the Community Integration Services and the 100 Pine Street Wellness and Recovery Center. Angela Deroshers from Maine Medical Center is working with both Common Ties Mental Health Services and Tri County Mental Health Services to work with individuals to explore and seek out employment and continuing education services. Please ask your Community Integration Worker or the staff at the Center about how to speak with Angela.
Medication Management
Medication Management services are available to eligible individuals in combination with our Community Integration Services, or as a stand alone service. Medication Management provides assessment, diagnosis, and treatment of psychiatric and co-occurring illnesses.
Integrated Care
Common Ties Mental Health Services continues its commitment to health and wellness for persons who experience a mental health illness through its second collaboration with St. Mary’s Health Systems to integrate behavioral health care in primary care settings.
Funded by the Maine Health Access Foundation, St. Mary’s Health system is expanding the use of a primary and behavioral health care team approach from one site at the B Street Health Center to two other sites at St. Mary’s Medical Associates and Central Maine Pediatrics.
Integrated Primary Care combines medical and behavioral health services to more fully address the spectrum of health issues through a coordinated plan of care.
St. Mary’s Health System provides an Advance Practice Nurse Practitioner and a Licensed Clinical Social Worker, while Common Ties Mental Health Services provides a Care Manager. Together, the team coordinates a complete plan of care and treatment.
Lisa Pepin is the Care Manager and assists each client with their human services needs coordinating access to community resources, complementing the medical care provided by the primary care practice.
