Referrals are accepted by all sources, including self referrals.  <strong>Please call us at 795-6710</strong> (it takes just a few minutes).Case Management assists people in accessing the services needed to address their mental health, medical, housing, substance abuse, vocational, and educational needs to name a few.  Case Managers will help consumers of mental health services navigate the service systems, achieve individualized treatment goals established with us and with their other providers if they wish, through education, modeling, resource referrals, collaboration, and communications.</p>
